Comfort We respect and understand that many patients experience dental anxiety. At our office, we do everything we can to make sure that your experience is an enjoyable one. By taking our time to sit with you and communicating clearly what you can expect during your treatment, we are often able to alleviate dental fear. We encourage you to bring headphones and music to listen to during your treatment. We also have pillows available for use. Honesty and Affordability We know that the cost of care can be confusing. In addition to offering in-office payment options to fit your budget, we also give you up-front cost information to the very best of our ability on your treatment ahead of time. We are not considered in-network with insurance at this time, but we do happily accept ALL PPO dental insurance. We also offer an in office only membership plan for those who may not have the benefit of insurance aid. We are happy to help you navigate your dental insurance and alternative payment options. First Visit Your initial new patient appointment will consist of examination, x-rays, oral cancer screening, and periodontal evaluation. Cleanings are scheduled at another time unless we have an opening that would be offered to you. For emergency appointments, treatment can be completed on the same day as the visit. However, a complex medical history or treatment plan could require a second appointment to provide treatment. Important: All patients under the age of 18 must be accompanied by a parent or guardian. X-Rays If your previous dentist has taken x-rays recently, you may request that they forward them to our office. We are looking for what we call a full set of x-rays or FMX for short. We are also looking for anything that was taken in the last year. This will allow for us to utilize those x-rays needed for the exam and not have to radiate you for additional x-rays. These must be brought to the initial exam or new ones will be taken. |
Please make sure that each of these forms are digitally filled out and signed. Remember, if you are younger then 18 years old, you must be accompanied by an adult. Please have your insurance card and drivers license ready to be copied when you enter the office. |
